Appropriation and allotment revision forms, 1965-[ongoing].

The Budget Division of the Dept. of Finance oversees budgeting procedure. These forms are filed by state agencies with the Budget Division to alter their appropriation or the current quarter's allotment or for program changes or other appropriation/allotment alterations. Information on these forms includes the fiscal year, the program number, fund number and amount of change. Also provided is the reason for revision, a breakdown of the revision such as increase or decrease in departmental receipts, federal funding, proration, etc. These forms are not filed for changes in future quarter allotments. The forms are signed by the department head, the budget officer and the governer.
